How can a minor child's name be legally changed in Ohio?

Ohio Revised Code Section 2717.01(B) provides the procedure for changing a minor child?s name. It is done by the Probate Court. Franklin County residents can access the necessary forms at http://www.franklincountyohio.gov/probate/forms/name_change/name_change_forms.cfm.
